Each association must provide the UEFA administration with a list of 12 players for the mini-tournaments and 14 players for the final tournament (surname, first name, shirt name, club and date of birth) as well as the surname and first name of the head coach. Two of these 12 or 14 players must be goalkeepers. The list of 12 (final tournament: 14) must be completed online by 15.00 CET the day before the team's first match in a mini-tournament and two days before the first match in the final tournament. A signed copy must be handed to the UEFA match delegate at the organisational meeting. In addition, this list must include confirmation from the team doctor that each player has undergone a medical examination to the extent provided for in the UEFA Medical Regulations; the team doctor is solely responsible for ensuring that these examinations have been duly performed.
For identification purposes, UEFA match delegates may make a visual check of each player participating in the competition. As a rule, one such visual check takes place at a mealtime at the team’s accommodation before a team's first match in each mini-tournament and in the final tournament.
Players named on the list of 12 (final tournament: 14) who become seriously injured or ill, to the extent that they cannot take part in a match, may be replaced as follows:

Qualifying competition: Outfield players may be replaced by another player before the start of a team’s first match. A replaced outfield player can take no further part in the mini-tournament in question.

Final tournament: Outfield players may be replaced by another player before their first match in the group stage. A replaced outfield player can take no further part in the final tournament.

All competition stages: Goalkeepers may be replaced at any time before the start of any match. Once an injured or ill goalkeeper is fit to be fielded again, he can resume his position in place of his nominated substitute, who must consequently be removed from the list. The return of the original goalkeeper must be announced to the UEFA administration no later than 23:59 CET the day before the next match in which the original goalkeeper is due to play.
The association concerned must submit the medical replacement form confirming the severity of the injury or illness, along with the necessary medical evidence, to the UEFA match delegate no later than 09:00 CET on the day of the match in question. If UEFA finds any of the information provided by the association to be inaccurate, the matter may be referred to the UEFA disciplinary bodies.
The associations are responsible for ensuring that the aforementioned provisions concerning player eligibility and player lists are observed.
