Each association must provide the UEFA administration with a list of 14 players (surname, first name, shirt name, club and date of birth) as well as the surname and first name of the head coach. Two of these 14 players must be goalkeepers. In addition, the list must include confirmation by the team doctor that all players have undergone the requested medical examination; the team doctor is solely responsible for ensuring that the requested players’ medical examination has been duly performed. This list must be completed online by 15.00 CET the day before the team's first match in the mini-tournament or a single match and two days before the first match in the final tournament. A signed copy of this list must be handed to the UEFA match delegate.
Upon submission of the signed medical replacement form to the UEFA match delegate, together with the necessary written medical evidence in one of UEFA’s official languages, outfield players or goalkeepers named on the final list of 14 players who become seriously injured or ill may be replaced as follows:

Qualifying competition: Outfield players may be replaced no later than three hours before the start of a team’s first match. A replaced outfield player can take no further part in the match or mini-tournament in question.

Final tournament: Outfield players may be replaced no later than three hours before their first match in the group stage. A replaced outfield player can take no further part in the final tournament.

All competition stages: Goalkeepers may be replaced at any time before the start of any match. Any such changes must be announced to the UEFA administration by 09:00 CET on the day of the match.
The associations are responsible for ensuring that the aforementioned provisions concerning player eligibility and lists of players are observed.
