For each round of the competition in which a club is participating, the club is responsible for submitting to UEFA a list of no more than 25 players, duly signed by the club and verified, validated and additionally signed by its association. Three of these players must be goalkeepers. This list must include the surname, first name, date of birth, nationality and national registration date of all players, as well as the surname and first name of the head coach. The club must complete this list online by 23.59 CET seven days prior to the start of a mini-tournament, a single match or the finals.
Only players who are duly registered with UEFA by means of the list of 25 players can validly serve pending suspensions.
The club bears the legal consequences of fielding a player who is not named on the player list, or who is otherwise not eligible to play.
Only 14 of the players on the list of 25 will be eligible to play in each round of the competition. The shortlist of 14 players, two of whom must be goalkeepers, must be submitted online by 15.00 CET the day before the first match of the team in each round of the qualifying phase, one day before each match in the round of 16 and quarter-finals and two days before the first match in the finals. A signed copy of this list must be handed to the UEFA match delegate at the organisational meeting. In addition, this list must include confirmation from the team doctor that each player has undergone a medical examination to the extent provided for in the UEFA Medical Regulations; the team doctor is solely responsible for ensuring that these examinations have been duly performed.
Players named on the list of 14 who become seriously injured or ill, to the extent that they cannot take part in a match, may be replaced as follows:

One outfield player may be replaced by another player from the list of 25 players before any match in the competition. A replaced outfield player can take no further part in that mini-tournament, single match or finals and no other outfield players may be replaced during that phase of the competition.

Goalkeepers may be replaced by another goalkeeper from the list of 25 players, or if that is not possible, by another goalkeeper not previously registered on the list of 25. Once an injured or ill goalkeeper is fit to be fielded again, he can resume his position in place of his nominated substitute.
The club concerned must submit the medical replacement form confirming the severity of the injury or illness, along with the necessary medical evidence, to the UEFA match delegate no later than 09:00 CET on the day of the match in question. If UEFA finds any of the information provided by the club to be inaccurate, the matter may be referred to the UEFA disciplinary bodies.
The clubs and their associations are responsible for ensuring that the aforementioned provisions concerning player eligibility and player lists are observed.
