The licensee must prepare and submit its club information to the UEFA administration and the CFCB by the deadline and in the form communicated by UEFA. Such club information comprises the licensee’s legal information (as defined in Article 60), its legal group structure (as defined in Article 63), its ownership and governance (as defined in Article 64) and its reporting perimeter (as defined in Article 66).
As part of its club information, the licensee must clearly identify any other football clubs over which any of the parties identified in its legal group structure, its ultimate controlling party, its ultimate beneficiary and any party with a significant influence or decisive influence over the licensee, or any of their key management personnel, have control or a significant influence or a decisive influence.
The licensee must prepare and submit its club information as indicated in Paragraph 79.01 and Paragraph 79.02 as at the annual accounting reference date of the reporting period ending in the calendar year that the UEFA club competitions commence. If not previously submitted to the UEFA administration and the CFCB, the licensee must also provide its club information for the immediately preceding two reporting periods. The licensor must confirm the completeness and accuracy of the licensee’s submission.
The licensee must provide information about any changes to the information indicated in Paragraph 79.01 and Paragraph 79.02 that occurred during the reporting period or during the period from the annual accounting reference date up to the submission of the information to the UEFA administration.
The licensee must confirm that its club information is complete, accurate and in compliance with these regulations. This must be evidenced by way of a brief statement and signature by the executive body/authorised signatories of the licensee.
